For each patient not entered on the Pathology sub-study:
1 H&E stained slide of the primary tumor
1 COPY of the pathology report of local pathologist
1 completely filled in pathology-form (Form P)
For each patient entered on the Pathology sub-study:
1 paraffin block of the primary tumor
1 H&E stained slide of the primary tumor
1 COPY of the pathology report of local pathologist
1 completely filled in pathology-form (Form P)
For ALL patients as of March 2005, if centers agree to participate in the central pathology reassessment:
1 paraffin block of the primary tumor
1 H&E stained slide of the primary tumor
