1 paraffin block of the primary tumor
1 paraffin block of normal tissue
1 H&E slide of the primary tumor
1 H&E slide of normal tissue
1 COPY of the pathology report of local pathologist
1 COPY of the completely filled in pathology-form (Form P)
(ORIGINAL Reports and Forms need to be DataFaxed and filed locally)
If patient has BILATERAL breast cancer, in addition:
1 paraffin block of the primary tumor of the second breast/side
1 H&E slide of the primary tumor of the second breast/side
1 COPY of the pathology report of local pathologist of second breast
1 COPY of the complete BILATERAL pathology-form (Form BP)
(ORIGINAL Reports and Forms need to be DataFaxed and filed locally)
